Title
Phthalate exposure and risk of diabetes mellitus: implications from a systematic review and meta-analysis
Review Question
(1) Is phthalate exposure associated with elevated risk of diabetes in humans? (2) If the answer of question one is positive, then which kind of phthalate correlates with diabetes? (3) Is phthalate exposure associated with insulin resistance in humans? If yes, which kind?
Study Type Included
Cohort study, case-control study, or cross-sectional study
Condition Being Studied
Diabetes mellitus (DM) has caused huge disease-economic burden and addressed deep public concern worldwide. It was reported that global prevalence of diabetes among adults was nearly 9.3% in the year of 2019, which indicated about 463 million adults had diabetes, and this number would increase to 700 million in 2045. Although many typical risk factors of DM have been identified, such as obesity, physical inactivity, unhealthy diet and family history of diabetes, risk factors and etiology of DM has not been fully revealed. In recent years, environmental endocrine disruptors, especially phthalate exposure, have been shown to be associated with risk of diabetes with inconsistent resutls. The potential hazardous effects of phthalate on DM and insulin resistance risk is not fully understood.
Participant
For studies regarding phthalate and diabetes, the study must contain case group of diabetic humans and control group without diabetes. For studies concerning phthalate and insulin resistance, the study included humans with or without diabetes.
Animal
Human Disease Modelled
Intervention
Exposure includes phthalate of its metabolites, as detected in human samples, such as blood, urine, hair, etc.
Comparator Control
For studies on phthalate and diabetes, the comparasion refers to effects of higher levels of phthalate or metabolites vs. that of lower levels on diabetes risk, presented as odds ratio (OR), risk ratio (RR) or hazard ratio (HR) with 95% confident interval (CI). For studies on phthalate and insulin resistance, the comparason refers to effects of higher levels of phthalate or metabolites vs. that of lower levels on insulin resistance.
Main Outcome
For studies on phthalate and diabetes, the outcome was defines as confirmed diabetes other than gestational diabetes diagnosed by doctors. For studies on phthalate and insulin resistance, the outcome referred to insulin resistance quantified with homeostasis model assessment (HOMA-IR) or its indices including serum fasting glucose, fasting insulin, or serum glycosylated hemoglobin. Measures of effect For studies on phthalate and diabetes, effect estimates included relative risks (RRs), odds ratios (ORs), or hazard ratios (HRs). For studies on phthalate and insulin resistance, effec estimates included ORs, RRs, HRs or correlation coefficients (sch as β-coefficient, etc.)
