The effect of phenylephrine on cerebral oxygenation and middle cerebral artery blood flow velocity in awake, adult, human participants: a systematic review protocol
Review Question
In awake, adult, human participants without neurological disease: (1)What is the effect of intravenous phenylephrine on cerebral oxygen saturation (COS)?, (2) What is the effect of intravenous phenylephrine on cardiac output and middle cerebral artery blood flow velocity (MCAv)? Other measures of cerebral perfusion will also be considered if included in the study design.
Study Type Included
This systematic review will consider both experimental and quasi-experimental study designs including randomized controlled trials, non-randomized controlled trials, before and after studies and interrupted time-series studies. Only studies published in English will be considered. Studies published since 1999 will be included.
Condition Being Studied
Cerebral perfusion, measured via cerebral oxygen saturation and middle cerebral artery blood flow velocity
Participant
The systematic review will consider studies of male and female human participants 18 years or older who are awake and not undergoing general, regional or sedation anesthesia. Participants with a history of intracranial neurological disease will be excluded.

Intervention
The systematic review will consider studies that evaluate the effect of intravenous phenylephrine administered as a bolus or continuous infusion
Comparator Control
This systemic review will consider studies that compare the intervention to pre and post administration, placebo, or other vasopressors such as epinephrine, norepinephrine and dopamine.
Main Outcome
This systematic review will include outcomes that measure the effect of phenylephrine on cerebral oxygen saturation and middle cerebral artery (MCA) blood flow velocity. Measures of effect Papers will, where possible be pooled in statistical meta-analysis using JBI SUMARI. Effect sizes will be expressed as either odds ratios (for dichotomous data), and weighted (or standardized) mean differences (for continuous data) and their 95% confidence intervals will be calculated for analysis. Heterogeneity will be assessed statistically using the standard chisquare and I² tests.
