- What are the outcomes following administration of papaverine in aneurysmal subarachnoid haemorrhage? Population (P): Patients with aneurysmal subarachnoid haemorrhage Intervention (I): Papaverine; topical or intravenous or intraarterial administration Comparator (C): wherever available the studies will be grouped for subgroup analysis with comparator as control or analogous other treatment modalities Outcome (O): prevention of vasospasm, intraoperative adverse effects, other adverse effects, mortality, infarct, neurological deficits, cranial nerve deficits Time frame (T): No time restriction Study design (S): RCT, Non-RCT, Clinical trial, Case series, Cohort study, Comparative study, Case reports and letter to the editor for qualitative synthesis. - Condition Being Studied
- Papaverine is commonly used to prevent vasospasm in aneurysmal subarachnoid haemorrhage patients. Papaverine is administered topically or intracisternal or intrarterial. The systematic review aims to evaluate the evidence on use of papaverine (topical/intraarterial) in preventing vasospasm. The review is focussed on different but related outcomes. The outcomes of interest in this study are vasospasm, mortality, adverse effects and complications.
- Participant
- Inclusion: All patients with aneurysmal subarachnoid haemorrhage receiving papaverine. Exclusion: Patients with pre-existing stroke, non-availability of data with respect to papaverine administration, use of drugs other than papaverine to prevent vasospasm. To study the adverse effect profile of topical papaverine, we will also include the case reports to appropriately identify the rare reported adverse events.

- Intervention
- Inclusion:Topical/intracisternal/intraarterial papaverine. Exclusion: drugs other than papaverine to prevent vasospasm. Studies where some other drugs are used with papaverine where papaverine is not the primary drug to prevent vasospasm
- Comparator Control
- Wherever available control population as saline or urokinase or other drugs will be used for comparison with papaverine. Such comparison will be done as subgroup analysis
- Main Outcome
- Two most important outcomes in this review are vasospasm and adverse effects. Vasospasm is defined by clinical and radiological findings. We will discuss the differences in these definitions in our reviews. Adverse effects as outcomes are defined as unintended harmful effect arising out of direct relationship with papaverine administration. Measures of effect In this review, we will use Odd's ratio for case control studies and relative risk for cohort studies. In the aggregated case reports the measures of effect will be assessed using descriptive statistics.
