Effectiveness, safety and cost-effectiveness of papain for treating chronic wounds: a systematic review 
Review Question
Is papain effective to treat chronic wounds? Is papain safe to treat chronic wounds? Is papain cost-effective to treat chronic wounds?
Study Type Included
Original published studies conducted in humans with the following types of design: 1. Randomized clinical trials; 2. Non-randomized clinical trials; 3. Prospective controlled cohort studies; 4. Economic evaluations: cost-benefit analyses, cost-utility studies, cost-effectiveness studies, cost-minimization studies and direct and/or indirect costs.
Condition Being Studied
Chronic wounds.
Participant
Adults or elderly with chronic wounds of any cause (such as pressure ulcers, venous leg ulcers, arterial ulcers, neurotrophic ulcers and foot ulcers in people with diabetes).
Animal
Human Disease Modelled
Intervention
Inclusion criteria: Papain in any concentration Exclusion criteria: Ongoing studies and study protocols.
Comparator Control
Placebo, standard care or alternative topical therapies.
Main Outcome
1.The proportion of chronic wounds completely healed. 2.Total epithelized area at the end of the intervention. 3. Percentage of the wound area healed. Measures of effect 1.The proportion of chronic wounds completely healed will be defined as 100% epithelization or skin closure without drainage. 2.Total epithelized area at the end of the intervention will be measured in cm-squared.
Outcome Measure
Additional Outcome
1. Time to complete wound healing. 2. Wound pain. 3. Wound complications: infection, necrosis. 4. Quality of life. 5. Adverse events. 6. Cost. Measures of effect 1. Time to complete wound healing evaluated with the register of the days or weeks.  2. Wound pain (measured by any validated scale). 3. Quality of life (measured by any validated scale).
