In patients with hyperuricemia, what is the effect of the xanthine-oxidase inhibitor allopurinol compared to placebo or other uric acid lowering drugs on cardiovascular outcomes and mortality?

Inclusion criteria: (Randomized) controlled trials, observational (cohort) studies (prospective or retrospective) Exclusion criteria: Case reports, N of one trials

Hyperuricemia
Participant
Inclusion criteria: Adult human patients with hyperuricemia, without renal disease, with or without atherosclerosis and with or without gout. Exclusion criteria: Animal studies, in vitro studies, in sillico studies, studies in patients with renal disease (MDRD<30 ml/min), studies in patients younger than 18 years and patients with tumor lysis syndrome (these patients are not comparable with cardiovascular hyperurecemic patients, since they have normal uric acid levels at start of treatment, often have hematological disease and/or receive immunosupressive comedication, which can interfere immunological mechanisms of hypersensitivity syndrome, the most important adverse event)

Inclusion criteria: Treatment with the xanthine oxidase inhibitor allopurinol, in any dose regime and treatment duration. Exclusion criteria: No allopurinol administered
Comparator Control
Inclusion criteria: Control group receiving placebo, no treatment, or other uric acid lowering drugs, for example benzbromaron Exclusion criteria: No appropriate control group present

Incidence of cardiovascular mortality Incidence of myocardial infarction Incidence of stroke Combined incidence of cardiovascular mortality, myocardial infarction and stroke Measures of effect For all outcomes, we will include data measured from randomisation to the last available follow-up (expected range 2-250 weeks)
