Review Question
To investigate whether xanthine oxidase inhibitors may ameliorate renal damage in individuals with manifested signs of chronic kidney disease (CKD). To assess the safety profile of these agents (risk of adverse effects) in this population.
Study Type Included
Any type of study could be included, except case reports or case series, reviews, editorials, letters, and studies not providing long-term data on the outcomes of interest.
Condition Being Studied
Chronic kidney disease (CKD) is a major public health problem as it is associated with elevated cardiovascular morbidity, mortality and hospitalizations.
Participant
Patients with CKD, which is defined according to the National Kidney Foundation-Kidney Disease Outcomes Quality Initiative (NKF KDOQI) guidelines by the presence of a reduced glomerular filtration rate (GFR) <90 mL/min/1.73 m-squared or the persistence of urinary abnormalities, such as albuminuria, proteinuria or hematuria in subjects with a GFR =90 mL/min/1.73 m-squared.

Intervention
Inclusion criteria: studies dealing with CKD individuals undergoing treatment with any xanthine-oxidase inhibitors (including but not limited to allopurinol, febuxostat or topiroxostat), providing information on the effects of these drugs on renal function/damage. Exclusion criteria; treatment with other drugs endowed with hypouricemic effects (e.g., losartan), or studies dealing with CKD patients on acute renal replacement therapy (e.g., acute dialysis or peritoneal dialysis). Studies where at least part of the population fulfilled the above criteria will be included in the review.
Comparator Control
Any type of comparator, including but not limited to placebo or standard care.
Main Outcome
End-stage kidney disease (ESKD), defined as the need for renal replacement therapy or kidney transplantation, or doubling of serum creatinine.
Outcome Measure
Additional Outcome
Changes in serum creatinine, proteinuria, albuminuria, and renal function (creatinine clearance/eGFR). Risk of adverse events will also be assessed.
