Sarcosine as an add-on treatment to antipsychotic medication for people with schizophrenia and other psychotic disorders: a systematic review and meta-analysis of randomized controlled trials
Review Question
To assess the effect of sarcosine, as an augmentation to antipsychotic usual treatment, in people affected by schizophrenia and other psychotic disorders.
Study Type Included
Published and unpublished randomised controlled trials (RCTs). No other restrictions will be imposed.
Condition Being Studied
Schizophrenia is an enduring mental disorder, with a prevalence of 1 % worldwide and similar incidence. Symptoms are usually categorised in two main domains: positive (e.g. delusions and hallucinations) and negative (e.g. lack of volition and apathy). According to symptom intensity and functioning impairment, patients suffering from schizophrenia present a trend alternating between stable periods and acute relapses. Moreover, schizophrenia often responds only partially to standard treatment, and negative symptoms are known to be particularly resistant to psychopharmacological medications, resulting in persistent residual symptoms and a poor prognosis. It is thus important to maximize the reinforcement of treatment effects to minimize residual symptoms and relapse frequencies, and to improve functional outcomes.
Participant
Adult patients (both men and women, 18 years of age and older) diagnosed with schizophrenia or other psychotic spectrum disorders. These disorders must have been defined according to standard operational diagnostic criteria (DSM or ICD, possibly updated to their latest versions).

Intervention
Sarcosine, administered via any route and pattern, and at any dose.
Comparator Control
Placebo, or any other agents that enhance N-methyl-D-aspartate (NMDA), but which function in a manner different to that of sarcosine. If multiple comparators are provided, we will give preference to placebo as the control group.
Main Outcome
Differences in pre-post treatment mean changes in psychotic symptom severity between sarcosine-treated and control patients, as assessed by the positive and negative syndrome scale (PANSS) global score, or any other validated rating scales that assess continuous psychotic symptom severity. If results on multiple symptom severity assessments are provided, we will give preference to PANSS scores. Measures of effect Early effect (greater than 72 hours but less than/equal to 2 weeks); acute effect (greater than 2 but less than/equal to 4 weeks); mid-term effect (greater than 4 but less than/equal to 6 weeks); long-term effect (greater than 6 weeks).
