Title
Hypertonic saline and mannitol in patients with Increased Intracranial Pressure after Stroke:A systematic Review and meta-analysis
Review Question
To evaluate the efficacy and safety of HTS solutions and mannitol in patients with elevated ICP secondary to stroke by examining the data from randomized controlled trials
Study Type Included
RCT are eligible for inclusion, case report, comments, cohort studies, animal studies are excluded.
Condition Being Studied
Increased ICP after stroke, including ischemic stroke as well as intracerebral hemmorage(ICH), leads to particularly high rate of neurological deterioration and mortality. Mannitol and hypertonic saline(HS) are both commonly used infusion as osmotherapy treatment in an effort to reduce the ICP. At present, the safety, effectiveness and scope of application of mannitol versus HS deserve further study.
Participant
Eligibility criteria for study inclusion consisted of full text available prospective, randomized clinical trials that compared HS and mannitol solutions administered intravenously in adult humans with evidence (clinical or measured) of elevated ICP secondary to stroke.Cross-over studies were eligible for inclusion, provided the initial treatment was randomized. The following exclusion criteria were applied: subarachnoid hemorrhage, case report, comments, cohort studies, animal studies, reviews and studies that included only pediatric patients (<16 years old).
Animal
Human Disease Modelled
Intervention
The intervention were hypertonic saline and mannitol.
Comparator Control
The intervention were hypertonic saline and mannitol.
Main Outcome
intracranial pressure (ICP) Measures of effect Additional outcomes were assessed using the mean differences(MD) or standardized mean difference (SMD) with the 95% CI.
Outcome Measure
Additional Outcome
1.cerebral perfusion pressure(CPP), 2.mean arterial pressure(MAP), 3.serum sodium level 4. plasma osmolality Measures of effect Additional outcomes were assessed using the mean differences(MD) or standardized mean difference (SMD) with the 95% CI.
