Safety of red yeast rice: An umbrella review of systematic reviews with meta-analysis
Review Question
(1) What adverse reactions can the red yeast rice cause? (2) How does red yeast rice impact on incidence of multiple adverse events? (3) What is the quality and credibility of the current evidence regarding the safety of red yeast rice?
Study Type Included
Only systematic reviews with meta-analysis of randomized controlled trials and (or) observational studies concerning the safety of red yeast rice will be included. Systematic reviews without data pooled-analysis for safety assessment, non-systematic literature and scoping reviews, systematic review and meta-analysis protocols, original research, animal studies, and unavailable studies will be excluded.
Condition Being Studied
Red yeast rice, also known as red yeast, is a traditional Chinese medicine made from rice, fermented with red Aspergillus, in the form of brownish red or purplish red rice grains. In recent years, a study found that during the fermentation process, red yeast rice produces a variety of secondary metabolites with good medical value. It is beneficial for cardiovascular health; osteoporosis; blood pressure and blood glucose regulation, etc. However, in past years, concerns have been raised about the safety of red yeast rice after some case reports alleging toxicity were published. Currently, many systematic literature reviews have been published; however, the conclusion concerning the safety of red yeast rice was different, which brings difficulties to clinical decision-making. Now, it is the time to summarize the evidence on the clinical safety of red yeast rice, assess the level and quality of the evidence, and draw more reliable conclusions.
Participant
A. Patients treated with red yeast rice (extracts) or combined therapies will be included in intervention groups, regardless of age, gender, race and severity of condition. B. Patients treated with placebos or other conventional therapies will be included in control groups, regardless of age, gender, race and severity of condition.

Patients in the treatment groups received the red yeast rice preparations alone or combined with conventional treatment, defining red yeast rice preparations as involving Xuezhikang interventions, Zhibituo interventions, Zhibitai interventions, Lovastatin interventions, or other relevant preparations or extracts.
Comparator Control
Patients in the control groups received the conventional treatment or placebos.
Main Outcome
Effects of red yeast rice on adverse reactions, including musculoskeletal disorders, abnormal liver function, gastrointestinal reaction, etc. Measures of effect The pooled estimate size with their 95% confidence intervals (CI) for red yeast rice and adverse reactions.
