- Study Type Included
- Randomized controlled trials included statin-intolerant patients in which at least one group is treated with red yeast rice with known content of Monacolin K.
- Condition Being Studied
- Indication for lipid lowering therapy either because of dyslipidemia or for cardiovascular risk reduction, but unable to tolerate statin therapy.
- Participant
- Participants with an indication for lipid lowering therapy either because of dyslipidemia or for cardiovascular risk reduction, who were unable to tolerate effective doses of at least one statin because of myalgia, myopathy, myositis, or rhabdomyolysis or any other adverse effects that resolved with statin discontinuation.

- Intervention
- At least one group must be treated with red yeast rice with known content of Monacolin K. Mode of delivery: oral. No lower or upper limit on total daily dose or frequency. Studies with co-interventions such as lifestyle therapy or concomitant (herbal) medication are included, as long as the co-interventions are available for intervention as well as control group.
- Comparator Control
- Inactive (placebo) or active control treatment.
- Main Outcome
- 1) Change in LDL-cholesterol. 2) Incidence of any adverse event. Measures of effect 1) Relative (%) change of the above mentioned lipid parameter(s) compared to baseline. If change score is not mentioned in a specific study and cannot be computed, we will try to retrieve it from authors or impute it from other lipid parameters. Since monacolin K is a statin-like substance, we assume that the timing of the effect on lipid metabolism is the same as in regular statin therapy, and is likely to have a lipid lowering effect within a few weeks. Therefore, we included studies with a treatment duration of at least 4 weeks. We collect the first series of lipid outcomes after 4 weeks treatment from each study. 2) Incidence (risk) between start and end of study.

- 1) Change in Total cholesterol, HDL-cholesterol, Triglycerides. Measures of effect 1) Relative (%) change of the above mentioned lipid parameter(s) compared to baseline.
