- Review Question
- What quantities of lutein/zeaxanthin are associated with normal healthy macular pigment optical density (MPOD) in the general North American adult population? Specific questions of interest: 1. Is there a minimum or incremental level of lutein/zeaxanthin intake associated with statistically significant positive change in MPOD? 2. Is there a minimum or incremental level of lutein/zeaxanthin intake associated with clinically important changes in MPOD? (clinically important changes in MPOD are those associated with reduced visual function (per results from ongoing review)) 3. Is there a dose response relationship for levels of intake of lutein/zeaxanthin associated with changes in MPOD? 4. Do results vary by dietary sources (food versus supplements) of lutein/zeaxanthin? 5. Does the association of levels of lutein/zeaxanthin with MPOD vary by age, sex, study location/country, risk of bias, supplements with or without meso-zeaxanthin, lutein/zeaxanthin supplements with or without other substances (e.g., zinc, omega-3 PUFA)?

- We will include the following study designs: • Randomized controlled trials • Controlled clinical trials • Comparative studies • Evaluation studies • Follow-up studies • Prospective studies • Crossover studies • Case-control studies • Matched-pair analyses • Interventional studies • Cross-sectional studies We will exclude reports with no original data, case reports, studies with less than 10 subjects, and non-English language publications.

- We will include studies conducted in a general adult population and asymptomatic of eye disease. However, we will allow populations that are at risk for macular changes, have early stage macular disease, cataracts, or Drusen in retina. We will exclude studies conducted among those with age-related macular degeneration, diabetic retinopathy, diabetes, children, pregnant or breastfeeding women, animals, or in vitro studies. Studies with mixed populations (those aged under and over 18 years old; pregnant and not pregnant) will be excluded unless data presented separately for population of interest.

- We will include studies that evaluate lutein/zeaxanthin supplements or dietary sources of lutein/zeaxanthin. We will exclude studies if we are unable to determine the dose of lutein/zeaxanthin.
- Comparator Control
- We will include comparisons to nothing, different doses of lutein/zeaxanthin, or different sources of lutein/zeaxanthin (e.g., supplements versus diet).
- Main Outcome
- The main outcome will be macular pigment optical density, as measured by Flicker, autofluorescence, or Raman spectroscopy. Measures of effect The measure of effect will be a mean difference or a relative risk.
