Condition Being Studied
Management of persistent patent ductus arteriosus (PDA) in preterm infants is a controversial topic in neonatal medicine. Previously studied management questions have centered around when and how to treat significant PDA. There have been a large number of published studies, meta-analysis, reviews focusing on different management approaches and outcomes. Ibuprofen is commonly used for prophylaxis and treatment of significant PDA, along with other therapeutic measures including indomethacin and acetaminophen. Ibuprofen is as effective and economical as indomethacin in closing PDA and in comparison, reduces the risk of associated complications including NEC and renal insufficiency. A question persists among the Neonatology sphere as to the variability of efficacy and associated risks of clinically relevant ibuprofen salts. At present, no direct randomized control trials have directly compared the two most commonly used IV ibuprofen salts, lysine and tromethamine (THAM). In current literature, neonates treated with ibuprofen lysine had improved renal outcomes when compared to neonates treated with indomethacin. Furthermore, IV ibuprofen lysine was suggested to be at least as effective as indomethacin for PDA closure, while IV ibuprofen THAM was associated with higher rates of comorbidity. This study aims to indirectly compare outcomes of treatment with both ibuprofen salts.
