- Title
- Tryptophan levels and its metabolites in the pathogenesis of postpartum blues and postpartum depression: a meta-analysis and systematic review
- Review Question
- How do serum tryptophan levels (and its metabolites) relate to postpartum psychiatric illness (in particular, postpartum blues and postpartum depression)?
- Study Type Included
- Designs eligible for inclusion include: cross-sectional, cohort studies and prospective case-control studies. Only human studies will be included.
- Condition Being Studied
- Peripartum period, pregnancy Postpartum depression / postpartum blues, maternal mental health Serum tryptophan, serum kynurenine, tryptophan metabolites
- Participant
- Adult women (18 years and over) who are pregnant and/or in the postpartum period (defined as the first year after delivery). We will exclude women with a pre-existing psychiatric illness (excluding Major Depressive Disorder) and those who are using psychiatric medications or medications known to affect tryptophan levels.
- Animal
- Human Disease Modelled
- Intervention
- Serum tryptophan levels (and kynurenine and other tryptophan metabolites) A change in serum levels will be measured either as: 1) A difference between antepartum and postpartum levels at 1 week, 1 month, or greater than 1 month 2) A comparison of serum levels in the postpartum period (at 1 week, 1 month or greater than 1 month) to a matched control group of non-depressed postpartum women
- Comparator Control

- Main Outcome
- Postpartum depression (defined as the onset of major depressive order, as per the DSM-V criteria, within the first 6 months postpartum) We will include studies that assess postpartum depression using symptoms questionnaires, screening measures or diagnostic assessments. Measures of effect
- Outcome Measure
- Additional Outcome
- Postpartum blues. Defined as feelings of sadness or moodiness with onset in the postpartum, but not meeting the criteria for major depressive disorder as per DSM-V. Diagnosed in studies using questionnaires, screening measures or diagnostic assessments.
