- Relationship between mild iodine deficiency (UIC: 100-150 μg/L) in pregnant women and thyroid function: a meta-analysis
- Review Question
- The thyroid hormone is an important regulatory hormone to maintain the normal growth and development of the fetus, especially the development of the brain and nervous system. Iodine is a necessary raw material for the synthesis of thyroid hormones. Once iodine deficiency occurs in pregnancy, it may affect the normal growth and development of the fetus. Especially the influence on the fetal nervous system and brain development cannot be ignored. In 2007, international organizations such as WHO/UNICEF/ICCIDD recommended the use of the median urinary iodine in pregnant women to assess the iodine nutritional status of pregnant women at the population level; the standard was a UIC of 150-250 μg/L for adequate iodine and < 150 μg /L for iodine deficiency. In non-pregnant women, the standard for iodine deficiency was <100 μg/L. As a result, it remains controversial in China whether mild iodine deficiency (UIC: 100-150 μg/L) affects thyroid function in pregnant women. We, therefore, intend to conduct a meta-analysis to further investigate the possible association between mild iodine deficiency and thyroid function tests in pregnant women.

- The thyroid hormone is an important regulatory hormone to maintain the normal growth and development of the fetus, especially the development of the brain and nervous system. Iodine is a necessary raw material for the synthesis of thyroid hormones. Once iodine deficiency occurs in pregnancy, it may affect the normal growth and development of the fetus. Especially the influence on the fetal nervous system and brain development cannot be ignored. In addition, as the fetus begins to produce thyroid hormone in the second and third trimesters of pregnancy, and as the kidney clearance rate of iodine in the woman increases, maternal iodine needs are further increased. A study in Denmark found that mild iodine deficiency did not have a severe or long-term effect on thyroid function in pregnant women. Nonetheless, as of now, it is not clearly understood whether mild iodine deficiency (UIC: 100-150 μg/L) affects thyroid function in pregnant women. The uncertainty about this association is invigorated by the existing literature; there are reports for and against the association between mild iodine deficiency and thyroid function.
- Participant
- The participants were pregnant women. The inclusion criteria :(i) Studies on the relationship between mild iodine deficiency and thyroid function tests in pregnant women; (ii) case–control study design; (iii) case group: pregnant women with mild iodine deficiency (specific cut-off values for adequate urinary iodine concentration were 100-150 μg/L); control group: pregnant women with adequate urinary iodine concentration (specific cut-off values for adequate urinary iodine concentration were 150-250 μg/L); (iv) complete data were available for statistical analysis, or data could be calculated based on the report's findings; (v) for studies that have been repeatedly published, the ones with the highest quality, the largest sample size, or the most detailed information were chosen; (vi) Only articles published in the English or Chinese language were considered. Exclusion criteria: (i) The study had no control group; (ii) literature with repeated reports, low quality, and unclear descriptions of specific data; (iii) reviews, conference abstracts, case reports, animal experiments, and other meta-analysis were not accepted.

So our primary study population was pregnant women with mild iodine deficiency (UIC: 100-150 μg/L).

- Control group: pregnant women with adequate urinary iodine concentration (specific cut-off values for adequate UIC were 150-250 μg/L)

- The outcome measures are thyroid function test related indicators (FT3, FT4, TSH, TPOAb and TGAb). Both quantitative and qualitative data are included.
