- Review Question
- The effect of dietary intake inulin-type fructans (ITF) on the human glycometabolism index is the question to be addressed. One previous meta-analysis conducted by Liu F was performed to assess the effects of ITF on human associated with metabolic abnormalities in dyslipidaemia, overweight or obesity, and type-2 diabetes. However, we still doubt their results. For one thing, we found some important available researches were not included in their review. What was more, one important index of glycometabolism, the HbA1c, was not studied in their systematic review. We will update the effect of inulin-type fructans on glycometabolism index, including fasting glucose, HbA1c, insulin, homeostasis model assessment of insulin resistance (HOMA-IR), HOMA-B and QUICKI, among the normal healthy, overweight or obesity, prediabetes, type-2 diabetes.

- Inulin, one kind of dietary prebiotic supplementation, predicted might have a profit on the glycometabolism index in some recent studies. However, in some researches, it showed no effect on the glucose control, and the previous systematic review conducted by Liu F also showed no significant change between the inulin-type fructans taker group and the control group. What was interesting, in our previous thesis research, we found that the inulin-type fructans showed an obvious effect on the glycometabolism, no matter in the type 2 diabetes and even in the healthy people. What was more, we found that some researches available were not included in their systematic review, and the important index like HbA1c was not studied.

- Inclusion criteria: trials involved human participants with age >18 years. Exclusion criteria: participants with infection disease conditions such as chronic hepatitis and inflammatory bowel disease.

- Inclusion criteria: intervention group receive a dietary prebiotic intervention, and the prebiotic was restricted to ITF (inulin, oligofructose, FOS and their mixtures). Exclusion criteria: 1. Interventions with the administration of probiotics (beneficial live micro-organisms) or synbiotics (a combination of pre- and probiotics) and other plant-derived carbohydrates such as arabino-xylan and S- glucan; 2. Interventions with interference of medication or other functional food; 3. Intervention time is more than one week.

- Inclusion criteria: control group received placebo maltodextrin or cellulose or no supplementation Exclusion criteria: control group received glucose or Xylitol.

- Fasting glucose, HbA1c and insulin Measures of effect The meta-analysis was conducted using RevMan software version 5.2 (Cochrane Collaboration, Oxford, UK). The effect sizes were expressed as the weighted mean difference (WMD) and 95% confidence interval (CI). The WMD was estimated by calculating the net change of the mean difference by subtracting the baseline value after treatment in the intervention and control groups. The standard deviation (SD) was calculated using the method described by Hozo et al [16] and Simental-Mendía et al [17]. If there were different reporting units for the indexes in the original studies, a unit conversion calculation was performed. Considering the included studies mainly performed according to health status, we categorized the participants into four groups: healthy, prediabetes and T2DM, overweight and obesity, and others. A random-effects model (using the DerSimonian-Laird method) and the generic inverse variance method were used to compensate for the heterogeneity of the studies in terms of the different groups. Interstudy heterogeneity was assessed using the Cochran Q test and I² index and was regarded as substantial if I² > 50% and P value was low (< 0.10).
