- Title
- Effect of lactic acid fermented foods on glycaemic control in diabetic adults: A Systemic Review and Meta-Analysis of Randomised Controlled Trials
- Review Question
- Effectiveness of lactic acid fermented foods on glycaemic control in diabetic adults.
- Study Type Included
- Randomised controlled trials (RCTs) Inclusion criteria: Randomised crossover study, randomized, double-blind, parallel-group, placebo-controlled trial. Exclusion criteria: Reviews, meta-analysis, qualitative study, non-experimental design studies.
- Condition Being Studied
- Diabetes mellitus is a chronic metabolic disorder, in which the body cannot respond to insulin effectively or there is insufficient insulin production from the pancreas, causing abnormally high blood glucose levels or hyperglycaemia. Type 2 diabetes mellitus (T2DM) is characterised by insulin resistance, where the body does not respond to insulin effectively. Prediabetes is a condition of intermediate hyperglycaemia but with blood glucose levels below the T2DM diagnostic threshold.
- Participant
- Inclusion criteria: Adults with type 2 diabetes mellitus or prediabetes (as diagnosed using any recognised diagnostic criteria). Exclusion criteria: Type 1 diabetes or gestational diabetes or studies involving adolescents (under 18 years of age) or animals (mice or rat).
- Animal
- Human Disease Modelled
- Intervention
- Intervention: Lactic acid bacteria (LAB) fermented foods, that undergo anaerobic process where sugars, such as glucose, into lactic acid, lactate, and other products. Inclusion criteria: Fermented foods include yoghurt, kefir, sauerkraut, kimchi and cheonggukjang, sourdough, kombucha, miso and natto, et cetera. Exclusion criteria: Foods that did not undergo fermentation, probiotic supplements.
- Comparator Control
- Placebo/control Inclusion criteria: non-exposed control group.
- Main Outcome
- Most important - Glycosylated Haemoglobin (HbA1c): An ideal glycaemic control HbA1c goal is levels below 7% (53 mmol/mol) without significant hypoglycaemia. Fasting plasma glucose (FPG) test uses enzymatic methods such as glucose oxidase and hexokinase. It measures the glucose levels in your blood after at least 8 hours of fasting. The ideal normal FPG range is considered to be 70-99mg/dL, with 100-125mg/dL indicating prediabetes and above 126mg/dL as type 2 diabetes mellitus. Fasting serum insulin is a test used to measure the amount of insulin in the body. It measures the insulin levels in your blood after at least 8 hours of fasting. The normal range is <25mIU/L (<174pmol/L). Homeostasis Model Assessment for Insulin Resistance (HOMA-IR) assesses the function of beta-cell by calculating the fasting glucose and insulin concentrations levels. The normal HOMA-IR level is 1 and the higher the HOMA-IR value, the more resistant the individual is to insulin.
- Outcome Measure
- Additional Outcome
- Lipid profile such as low-density lipoproteins (LDL), high-density lipoproteins (HDL), Triglycerides (TG), Total Cholesterol (TC) and inflammatory markers such as Interleukin 6 (IL-6) and tumour necrosis factor alpha (TNF-a). Other markers such as systolic blood pressure (SBP), diastolic blood pressure (DBP) and body mass index (BMI).
