Title
Serum and broncho alveolar lavage galactomannan antigen detection for the diagnosis of invasive pulmonary aspergillosis in patients with hematological malignancies
Review Question
1. What is the optimal cut-off value of the serum galactomannan OD index for the detection of pulmonary aspergillosis and what is the sensitivity and specificity? 2. What is the optimal cut-off value of the BAL galactomannan OD index for the detection of pulmonary aspergillosis and what is the sensitivity and specificity? 3. Is it useful to test the serum galactomannan bi-weakly for early screening on pulmonary aspergillosis? 4. Is it useful to test the serum galactomannan bi-weakly during anti-fungal treatment to measure response to therapy?

Condition Being Studied
Patients with hematological malignancies are often treated with chemotherapy, thus making them immunocompromised. This greatly increases their risk for opportunistic infections such as invasive pulmonary aspergillosis which is a major cause of mortality and morbidity in this patient group. Early diagnosis and treatment are therefore an important cornerstone in the survival of these patients. Invasive pulmonary aspergillosis is a fungal infection of the lungs and is mostly caused bij the species A. fumigatus. It shows symptoms of fever, chest pain, shortness of breath, coughing and hemoptysis.
Participant
The population being studied will be 18+ patients with an hematological malignancy suspected of having invasive pulmonary aspergillosis.
Animal
Human Disease Modelled
Intervention
We will review the usefulness of the serum and broncho alveolar lavage galactomannan antigen assay in the diagnosis of invasive pulmonary aspergillosis. We will asses the sensitivity and specificity for different cut-off values of the OD-index.
Comparator Control
Not applicable
Main Outcome
Sensitivity and specificity of the serum and BAL galactomannan OD index at different cut-off values.
