Review Question
What is the effect of N-acetyl cysteine on mortality in patients with non-acetaminophen acute liver failure? What is the benefit of N-acetyl cysteine on hospital stay and clinical outcomes in non-acetaminophen acute liver failure?
Study Type Included
We will include case studies/series, cross-sectional studies, cohort studies, and RCTs using N-acetyl cysteine for the management of acute liver failure. Studies published in English or with an English translation will be included. We will exclude editorials, comments, and studies without adequate data of interest.
Condition Being Studied
Acute liver failure is characterized by acute liver injury, hepatic encephalopathy, and an elevated prothrombin time/international normalized ratio (INR). Causes of acute liver failure include acetaminophen and non-acetaminophen like other drugs, alcohol, viruses, autoimmune hepatitis, etc. Non-acetaminophen causes of acute liver failure account for just over half of the total cases of acute liver failure. N-acetyl cysteine is a free radical scavenger. It contains a thiol agent that scavenges free oxygen radicals to restore cellular glutathione reserve. Glutathione prevents hepatic damage by combining with reactive metabolites.

Inclusion: Patients with acute liver failure due to causes other than acetaminophen who received N-acetyl cysteine during management Exclusion: Patients with acute liver failure due to acetaminophen ingestion (? Coexisting acetaminophen and other conditions)

N-acetyl cysteine in patients with acute liver failure due to causes other than acetaminophen
Comparator Control
Patients with non-acetaminophen induced acute liver failure who did not receive N-acetyl cysteine during management
Main Outcome
Differences between recovery/ resolution, mortality, and duration of hospital stay in patients receiving and not receiving N-acetyl cysteine with acute liver failure Measures of effect OR/RR as appropriate will be used.

Additional Outcome
Subgroup analysis based on the type of studies and sample number will be used as appropriate. Measures of effect OR/RR as appropriate will be used
