- Review Question
- To compare the effects and safety of misoprostol versus ergometrine-oxytocin for PPH prevention, and explore the potential drug using in low-resource settings.
- Study Type Included
- All randomized controlled trials (RCTs) related to comparison of prophylactic misoprostol and ergometrine-oxytocin for PPH in the third stage of labor would be included. Non-randomized trials, such as descriptive study, clinical control study and unclearly randomized trials would be excluded.
- Condition Being Studied
- Postpartum hemorrhage (PPH) is a major cause of maternal mortality, accounting for more than 100,000 maternal deaths per year. PPH also results in long-term disabilities and severe maternal morbidities. An estimated 287 000 maternal deaths occurred worldwide in 2010, among those pregnant women in sub-Saharan Africa (56%) and southern Asia (29%) accounted for 85% of the global burden in 2010. The United Nations aims to reduce maternal mortality by 75% by 2015. Thus far, the progress is slow in developing countries . Uterine atony is a main cause of PPH in pregnant women. The World Health Organization(WHO)2012 and International Federation of Gynecology and Obstetrics (FIGO)2012 guidelines have recommended that routine administration of uterotonics.
- Participant
- The parturients were misoprotol or ergometrine-oxytocin administered in the third stage of labor for preventing PPH, whether the delivery mode was cearean section or vaginal delivery.

- Intervention
- All randomized controlled trials (RCTs) related to comparison of prophylactic misoprostol and ergometrine-oxytocin for PPH in the third stage of labor were included. Misoprostol was the intervention, and can be administrated by oral, rectal, sublingual, and vaginal ways. The dose of misoprostol wasnot limited.
- Comparator Control
- Ergometrine-oxytocin was the control group, and administrated as syntometrine® or a combination of fixed dose of 0.5mg ergometrine and 5 IU oxytocin. Studies comparing misoprostol and ergometrine-oxytocin for treating PPH, or using the combination of ergometrine and oxytocin with variable indigents would be also excluded.
- Main Outcome
- In this review, we defined PPH for blood loss of >500 ml within 24 hours of delivery, severe PPH for blood loss of 1000 ml or more within 24 hours of delivery, and use of additional uterotonic therapy as primary outcomes. Measures of effect The results would be recorded during 24 hours of delivery

- The mean blood loss (ml), mean haemoglobin change between pre-delivery and post-delivery (g/l) and need for manual removal of placenta were deemed as secondary outcomes. The severe materal morbidities and incidences of adverse events would also recorded.
