- Review Question
- What are the human clinical trial outcomes of different nonantibiotic alternatives including: 1. cranberry 2. D-mannose 3. NSAIDs in preventing or treating uncomplicated UTIs in adult, non-pregnant women?
- Study Type Included
- Just needs to be a human clinical trial in women, there are no restrictions to the type of study. This is because we do not want to limit the scope of trials.
- Condition Being Studied
- Urinary tract Infections (UTIs) are most common in women, comprising nearly 25% of all infections. Approximately 50 to 60% of women will report a UTI at least once in their lifetime, with one in three women experiencing a symptomatic UTI treated with antibiotics by age 24. Of these women, 20–40% will experience recurrent UTI (r-UTI). To treat UTIs, antibiotics are typically prescribed for uncomplicated bacterial cystitis. However, in many countries antibiotic resistance has significantly increased towards these first-line antimicrobial drugs and developed multi- and pan-drug resistant strains. Hence, there is a need to investigate other nonantibiotic alternatives for preventing or treating UTIs. This review hopes to provide evidence from human clinical trials of whether nonantibiotic treatments were effective in preventing or treating uncomplicated UTIs in adult, non-pregnant women.

- Inclusion: Adult, non-pregnant women (<u>></u> 18 years old) with an uncomplicated UTI Exclusion: Pregnant women, children, men with a UTI

- Human Disease Modelled
- Intervention
- Inclusion: Cranberry (any cranberry formulations in a capsule or drink form) , D-mannose (any D-mannose formulations in a capsule or drink form) or non-steroidal anti-inflammatory drugs (NSAIDs)
- Comparator Control
- Must compare a cranberry, D-mannose or NSAID to a control (placebo, lower dosage of nonantibiotic treatment, no intervention etc.)
- Main Outcome
- Primary outcomes assessed include: 1. The number of symptomatic UTIs obtained during treatment period 2. Symptom resolution Secondary outcomes assessed include: 1. Time to first recurrence 2. Change in antibiotic consumption Measures of effect Number needed to prevent or treat UTIs
