- Review Question
- For patients with central nervous system (CNS) aspergillosis, proven by histopathology and/or radiographic imaging, what is the diagnostic accuracy (sensitivity and specificity) of galactomannan detection in the cerebrospinal fluid (CSF)?
- Study Type Included
- Inclusion: prospective and retrospective cohort studies using any galactomannan assay on CSF samples, either alone or in combination with other testing modalities. Exclusion: case reports; subject reviews; case series with < 5 CNS aspergillosis patients.

- CNS infections, aspergillosis, diagnostic test accuracy
- Participant
- Inclusion: immunocompetent and immunocompromised patients, adult and pediatric. Exclusion: non-humans, allergic aspergillosis, invasive pulmonary aspergillosis, aspergillosis outside the CNS.

- Intervention
- Inclusion: CSF galactomannan used singularly or in combination with other tests for the diagnosis of CNS aspergillosis. Exclusion: 1, 3-beta-D-glucan alone, Aspergillus PCR alone, or other non-galactomannan-based diagnostic techniques used alone in the diagnosis of CNS aspergillosis.
- Comparator Control
- The 2019 EORTC/MSG diagnostic criteria, which classifies patients into three groups as: proven, probably, or possibly suffering from invasive aspergillosis. To construct 2x2 tables, we defined possible aspergillosis as “no CNS aspergillosis”; and proven and probable aspergillosis as “having CNS aspergillosis”. - The primary goal of the study is the assessment of the accuracy of galactomannan testing of CSF in the diagnosis of CNS aspergillosis at different cut-offs for test positivity.
