- Is Serum Beta-D-Glucan Assay better than Galactomannan Antigen Assay for diagnosing invasive fungal infections in children?
- Study Type Included
- Diagnostic Cross sectional studies will be included.
- Condition Being Studied
- Due to the rising number of immunocompromised children at risk of developing invasive fungal infections (IFI), this condition continues to be associated with considerable morbidity, mortality and financial burden. Children with hematologic malignancies, congenital or acquired immunodeficiency, those receiving immunosupressants, premature neonates and children admitted to intensive care units are the major subsets at risk of developing IFIs. The two most common pediatric IFIs are invasive candidiasis (IC) and invasive aspergillosis (IA). Despite the availability of new antifungal agents, their substantially different pharmacokinetics in children and adults mandates the use of age-adapted management approaches to treat IFI in pediatric population. One of the critical pre-requisites for the appropriate initiation of treatment for IFI is its prompt and accurate diagnosis. Accurate diagnosis will also prevent the unnecessary use of potentially toxic and expensive antifungal drugs in patients with clinically suspected IFI. Conventional diagnostic modalities of culture and microscopy are limited by a low sensitivity, long turnaround time, the need for invasive sampling and the frequent contamination of fungal cultures. IFI diagnosis is even more challenging in children since clinical signs and symptoms are often non specific and performing invasive sampling is a point of contention.
- Participant
- Inclusion criteria: All children <18 years of age with the suspicion of Invasive Fungal Infections(IFIs). Children at high risk of IFIs are those suffering from primary or secondary immuno-deficiencies , Haematological malignancies, Prematurity or prolonged illness with intensive care interventions will be included.

- Intervention
- The Studies in which both B-D-Glucan and Galactomannan Antigen assay have been used to diagnose IFI will be included. The studies in which only one of the two tests has been used will be excluded.
- Comparator Control
- The reference test for diagnosing IFI should be as per the EORTC/MSG guidelines which uses Microscopic analysis or culture from a sterile site, Blood culture and yielding mould or fungus for a positive diagnosis.
- Main Outcome
- Primary outcomes will include Sensitivity and Specificity of Serum Beta-D-Glucan and Galactomannan Antigen assay for the diagnosis of IFIs in children with the EORTC/MSG criteria for Proven IFIs used as the gold standard. The Data for sensitivity and specificity will be pooled to ascertain which of the two tests is better or whether both should be combined to achieve the maximum diagnostic accuracy.
