Predictive value of abdominal drain amylase for postoperative pancreatic fistula following gastric resection: a systematic review and meta-analysis
Review Question
To determine the diagnostic accuracy of drain amylase activity in the early prediction of pancreatic fistula development after oncologic, pancreas sparing, gastric resections.
Study Type Included
We plan to include all studies that evaluate the diagnostic accuracy of drain amylase levels for the diagnosis or prediction of postoperative pancreatic fistula (POPF) in patients with the diagnosis of gastric cancer who have undergone any type of gastric resection with intra-operatively inserted abdominal drain. We will include both laparoscopic and open surgery cases. There will be no age restriction. Articles in any language will be eligible for inclusion. Articles in languages other than English will be translated. We will exclude studies without original or insufficient data (insufficient data to construct 2x2 contingency table even after coresspondence to authors (amylase levels with cutoffs vs. occurrence of POPF in test positive and negative groups), abstracts, letters, editorials, empty reviews, case reports, etc.).
Condition Being Studied
Gastric resection with lymphadenectomy remains main principle of treatment for gastric cancer. Radical dissection of lymph nodes can lead to collateral damage to the pancreas in form of pancreatitis, peripancreatic abscess, and postoperative pancreatic fistula (POPF). Despite of questionable benefits of abdominal drainage, drainage fluid composition could be an early sign of possible complications. There is evidence of to support value of drainage amylase activity on the first postoperative day after pancreatic resections to predict the occurrence of POPF and we believe the same principle can be applied to gastric resections where the integrity of pancreas is preserved. Therefore, clinical relevance of the test is to direct postoperative management either to fast-track recovery protocol or more suspenseful traditional approach.
Participant
Patients who have undergone oncologic gastric resection with preservation of pancreas.

Intervention
Drain amylase level assessments.
Comparator Control
Participants with no POPF, or Grade A POPF (according to ISGPF) will be regarded as controls.
Main Outcome
Diagnostic test accuracy. Measures of effect Diagnostic odds ratio (DOR) with positive and negative likelihood ratio (PLR, NLR).

Risk factors for the development of clinically relevant postoperative pancreatic fistula Grade B/C POPF (according to ISGPF). Measures of effect Risk ratio (RR)
