Serum amylase in predicting mortality after acute paraquat poisoning: a meta-analysis
Review Question
This meta-analysis aimed to explore the effects of serum amylase as a predictor of mortality in patients with paraquat (PQ) poisoning.
Study Type Included
Studies were included if they met the following criteria: all prospective and retrospective full-text articles comparing the diagnostic performance of serum amylase with prognosis in PQ poisoning patients; all the subjects included in the study been given a definitive diagnosis; and serum amylase are available to calculate odds ratio (OR) and its 95% confidence interval (CI). Studies were excluded due to the following reasons: duplicated or overlapped studies; abstracts, comments, case reports, reviews, meta-analyses, and conference papers.
Condition Being Studied
Paraquat (PQ, 1, 1′-dimethyl-4, 4′-bipyridinium dichloride) is a quaternary nitrogen herbicide used worldwide that exerts strong toxic effects on humans and animals. Being inexpensive and easily accessible, it has become the herbicide with the highest acute poisoning mortality rate. Numerous treatment strategies for managing poisoning have been proposed, including extracorporeal elimination, immunosuppressants, and antioxidants, but the mortality rate remains high even in centers that employ these treatments. If not treated in time, PQ accumulation in the body leads to fulminant organ failure, including pulmonary edema, cardiac, kidney, and hepatic failure. Therefore, confirming the PQ-poisoning diagnosis and risk assessment in a timely manner is particularly important. Pancreatic enzymes were found to be elevated in patients with severe poisoning and pancreatitis was observed at autopsy of PQ-poisoned patients. Recently, serum amylase was been used to predict the prognosis and mortality of patients with PQ poisoning. However, as to whether serum amylase possess adequate prognostic accuracy for PQ poisoning patients. Therefore, we conducted a meta-analysis to evaluate the value of arterial lactate as a predictor of mortality in patients with PQ poisoning.

Comparator Control
Serum amylase levels will be compared between surviving and non-surviving patients with PQ poisoning.
