Title
Serum creatinine in predicting mortality after paraquat poisoning :a meta-analysis
Review Question
we have ample data to explore the potential of serum creatinine as a predictor of mortality in patients with paraquat (PQ) poisoning.
Study Type Included
Observational studies (prospective studies and retrospective studies) will be included.
Condition Being Studied
Paraquat (PQ, 1, 1′-dimethyl-4, 4′-bipyridinium dichloride) is a quaternary nitrogen herbicide used worldwide that exerts strong toxic effects on humans and animals. Being inexpensive and easily accessible, it has become the herbicide with the highest acute poisoning mortality rate. Management of PQ poisoning has remained predominantly supportive and the results for the treatment of PQ poisoning, including immunosuppressive therapy, prolonged extracorporeal elimination and lung transplantation, need to been further confirmed. If not treated in time, PQ accumulation in the body leads to fulminant organ failure, including pulmonary edema, cardiac, kidney, and hepatic failure. Therefore, confirming the PQ-poisoning diagnosis and risk assessment in a timely manner is particularly important. PQ induces acute tubular necrosis due to direct toxicity to the proximal tubule. Other factors influencing the development of acute kidney injury include hypoperfusion from hypovolaemia and/or hypotension and direct glomerular injury. Recently, serum creatinine was been used to predict the prognosis and mortality of patients with PQ poisoning, although its prognostic accuracy for patients with PQ poisoning has not been fully determined. Therefore, we will conduct this a meta-analysis to evaluate the value of serum creatinine as a predictor of mortality in patients with PQ poisoning.
