Title
Vaginal fluid urea and creatinine in the diagnosis of premature rupture of membranes. A systematic review
Review Question
Are urea and creatinine in vaginal sample reliable to diagnose the premature rupture of membranes?
Study Type Included
All published case-control and cohort studies.
Condition Being Studied
Premature rupture of membranes (PROM) occurs in 10% of all pregnancies and about 2–4% of preterm pregnancies. PROM refers to rupture of the fetal membranes prior to the onset of labor and can occur at any gestational age. PROM is associated with adverse complications for the mother and the neonate such as chorioamnionitis, placental abruption, preterm birth and postpartum infection. Therefore, the diagnosis of PROM is of particular importance.
Participant
Pregnant women who are complaining of vaginal fluid leakage between 20 and 41 weeks of gestation.
Animal
Human Disease Modelled
Intervention
Urea plays an important role in the metabolism of nitrogen-containing compounds in the urine. Creatinine is a break-down product of creatinine phosphate in muscles and is usually produced at a fairly constant rate and is mainly filtered out of the blood by kidneys. Fetal urine is the most important source of amniotic fluid in the second and third trimester of pregnancy. Thus urea and creatinine of vaginal fluid may be helpful in diagnosing of PROM.
Comparator Control
The control group are women without PROM.
Main Outcome
Mean value of urea measured by enzymatic urease method and mean value of creatinine measured by Jaffe method. Measures of effect Second and third trimester.
Outcome Measure
Additional Outcome
None Measures of effect Neonatal period and postpartum period
Study Method
Diagnostic, Meta-analysis, Prevention, Systematic review
