Title
Prosthetic Hip Associated Cobalt Toxicity: a systematic review
Review Question
The present study aims to perform a systematic review of case reports describing cobalt intoxication after any type of hip arthroplasty to define the symptoms of cobalt toxicity and to determine a level of cobalt concentration when toxicity might occur
Study Type Included
Case reports of patients presenting with cobalt intoxication after hip arthroplasty, published in Dutch, English, German of Spanish.
Condition Being Studied
An increase in blood serum metal ion levels is seen after implantation of all MoM hip prosthesis due to release from the metal surface directly, more so during articulation and corrosion of the bearing surfaces. Corrosion of the bearing surface also occurs when a metal (cobalt-chromium) component is used during revision of a fractured ceramic head of fractured ceramig acetabular liner. The continuous exposure to metal ions can result in soft tissue reactions and raised metal ion concentrations in blood, serum and urine. Systematic complaints in patients with MoM arthroplasty is als known as Prosthetic Hip Associated Cobalt Toxicity and may lead to a variety of symptoms: neuro-ocular-, cardio- and thyroid toxicity. It is currently unknown which of these symptoms is most reported and at what level of cobalt concentrations symptoms occer.
Participant
Patient with any type of hip arthroplasty (metal-on-metal, ceramic-on-ceramic, ceramic-on-polyethylene, metal-on-polyethyelene)
Animal
Human Disease Modelled
Intervention
Presentation with cobalt toxicity
Comparator Control
This review does not focus on another intervention or a non-exposed controle group.
Main Outcome
The primary outcome are the reported symptoms of cobalt intoxication and the threshold concentration at which symptoms occur. Measures of effect Percentages in which symptoms occur, and a threshold concentration at which symptoms occur
Outcome Measure
Additional Outcome
The secondary outcome is the clinical outcome after treatment of cobalt intoxication Measures of effect Secondary outcomes will be shown descriptively
