- Study Type Included
- All peer-reviewed cohort studies that discuss the association between chronic lead and/or cadmium exposure and cardiovascular outcomes, will be included.
- Condition Being Studied
- Cardiovascular diseases have remained the leading cause of mortality worldwide for the last few decades (Roth et al. 2020). Several factors are contributing to cardiovascular health including patient demographics, related medical conditions and risk factors, and social determinants of health (Anonymous 2014; Powell-Wiley et al. 2022). These social determinants of health include environmental factors such as exposure to environmental metals (lead, cadmium, mercury, and copper). Growing evidence points toward its effect on cardiovascular health outcomes including coronary artery disease and stroke (Chowdhury et al. 2018; Nigra et al. 2016; Yang et al. 2021).
- Participant
- Inclusion criteria: • Articles discussing the association between chronic lead and/or cadmium exposure and cardiovascular outcomes (coronary artery disease, peripheral artery disease, and heart failure). • Study design will be restricted to cohort studies for assessment of the causal relationship and to reduce the heterogeneity as a result of various study designs. • No restriction on the date of publication or country. • No restriction on population age, sex, race, or country. • No restriction on the method of reporting the outcomes. • No restriction on the language of publication. Exclusion criteria: • Concurrent or exclusive exposure to other heavy metals (e.g., cobalt, mercury, aluminum, arsenic) will be excluded. • Non-peer reviewed articles will be excluded, including correspondence, letters to editor, opinions, comments, and author response. • Abstract-only articles, reviews, theses, books, or conference articles/proceedings. • In-vitro or animal studies. • Duplicate or overlapping data. • Articles with unreachable or unavailable full texts.

- Chronic lead and/or cadmium exposure. This exposure should be measured in humans either using serum, urine, hair, or other methods.
- Comparator Control
- Comparators will differ across studies depending on the study design and the method of measuring the heavy metal in the blood. Control is defined as participants from those studies who were not exposed to lead or cadmium.
- Main Outcome
- - Cardiovascular diseases, including coronary artery disease, peripheral artery disease, and heart failure. Other cardiovascular outcomes (e.g., stroke, arrhythmia) will be excluded. Outcomes reporting could be self-reported, extracted from medical records, or collected by the investigators.

- - If the number of studies reporting adjusted odds is sufficient to conduct a meta-analysis, adjusted odds of cardiovascular diseases, including coronary artery disease, peripheral artery disease, and heart failure will be estimated.
